Housing Navigator Job Description

Compensation: $17 - $18 per hour

Job Summary:

The Housing Navigator will play a pivotal role in facilitating housing placements for clients, focusing on efficient administrative processes, client coordination, and effective communication. This part-time position is vital for ensuring clients receive the housing support they need while maintaining adherence to all legal and regulatory requirements.

Key Responsibilities:

  1. Administrative Efficiency: Perform administrative tasks with a focus on efficient housing placement, primarily through extensive phone communication.

  2. Client Placement Coordination: Coordinate facility administrator assessments to facilitate the placement of clients into suitable housing.

  3. Documentation and Correspondence:
    - Assist in housing navigation-related correspondence and ensure timely uploading into the Apricot case management system.
    -Maintain real-time and accurate notes in Apricot, contributing to seamless and timely housing transitions.

  4. Technology Utilization:
    Utilize OpenPhone for troubleshooting assistance, ensuring effective communication and problem resolution.

  5. Information Handling:
    - Address frequently asked questions about housing availability, escalating complex inquiries to LES or Supervisor when necessary.
    - Ensure confidentiality of patient and agency information in accordance with HIPAA regulations.

  • Case Management: Ability to manage multiple cases simultaneously, track progress, coordinate facility assessments for placement, and ensure follow-up.

  • Client Advocacy: Advocate effectively for clients' needs and rights, ensuring access to necessary resources.

  • Communication Skills: Strong verbal and written communication skills for interacting with clients, facility administrators, and families; clearly convey information and instructions.

  • Cultural Competence: Understand and respect diverse backgrounds and needs, tailoring approaches to fit different cultural contexts.

  • Empathy and Sensitivity: Demonstrate understanding and compassion for participants' situations; exhibit excellent interpersonal communication skills when managing sensitive and confidential situations with professionalism.

  • Knowledge of Legal and Regulatory Frameworks: Familiarity with relevant laws, regulations, and policies related to housing and tenants' rights to provide accurate guidance and support.

  • Organizational Skills: Excellent organizational abilities to manage documentation, track applications, and coordinate effectively between various parties.

  • Problem-Solving: Ability to think critically and creatively to overcome obstacles and find solutions to housing-related issues.

  • Utilizing Provided Housing Resources: Understand available housing options using the ALW Facility List.

Qualifications:
GED/High School Diploma required.
Previous experience working in an office or administrative setting.
(Preferred) Previous experience at a Care Coordinating Agency (CCA) for the Assisted Living Waiver Program.
