Providing a continuum for Homeless Patients

Most provisions take effect on January 1, 2019
Effective July 1, 2019 providers need a written plan to coordinate with community partners and keep homeless patient logs.
Requirements:
Discharge plan prepared for each patient.
Referral for follow-up care (medical/behavioral)
Connecting patient with available community resources, treatment, shelter, and other supportive services.
Solutions:
Responsive intake for all eligible clients.
Case Management, Social Services and Home Health Patient Care.
Assist patients with enrollment in Medi-Cal program for placement in Assisted Living Waiver Program (if qualified)
Medical Respite Services available to contracted hospitals and medical groups only.
