Position Summary
All Hours Adult Care, SPC (AHAC), a DHCS-listed Care Coordination Agency for California's Assisted Living Waiver (ALW) Program, is accepting applications from California-licensed RNs for an evergreen independent contractor pool supporting Initial Assessments (IAs) and Reassessments (RAs).
Compensation
Competitive per-assessment compensation. Exact virtual and in-person rates are disclosed in AHAC's secure application and confirmed in writing before services begin. Volume and earnings are not guaranteed.
Contracted Services
- Conduct Initial Assessments and semiannual Reassessments using AHAC- and DHCS-approved tools, procedures, and secure systems.
- Interview participants and, as applicable, authorized representatives, caregivers, facility staff, and other care-team members.
- Assess participant-specific medical, functional, cognitive, behavioral, environmental, and support needs within the RN's scope.
- Develop or update the participant-centered Individualized Service Plan and prepare complete information for DHCS review and final level-of-care determination.
- Submit accurate, individualized records in MedCompass and any other AHAC-designated secure system, generally the same day or within 48 hours; protect confidentiality and communicate professionally.
MedCompass Completion Requirement
For compensation purposes, an assessment is complete only after the clinical service and all required documentation are timely submitted in the DHCS MedCompass platform and any designated AHAC system under the written agreement. Required MedCompass training is provided during onboarding.
Required Qualifications
- Current, unsuspended, unrevoked California Registered Nurse license.
- At least 1,000 hours in an acute-care setting with patients having similar care needs, or at least 2,000 hours in a home or home-health setting with patients having similar care needs. Only one pathway is required.
- Strong clinical assessment, care planning, documentation, time-management, and communication skills.
- Ability to complete required AHAC, ALW, privacy, security, and MedCompass training and to support selected service areas through travel and/or secure telehealth technology.
Preferred Experience and Communication
ALW or other HCBS programs, adult or geriatric nursing, case management, care coordination, RCFE/ARF settings, ISPs, home-based nursing, telehealth, or comparable documentation. ASL proficiency is preferred and may be required for designated assignments.
Capacity, Work Invitations, and Service Area
Applicants state a realistic service area, availability, and capacity. When demand supports it, high-capacity contractors may be offered approximately 50-100 assessments per month; lower capacities are welcome. Contractors may accept or decline a Work Invitation. Once accepted, it is binding under the signed agreement and must be completed within its scope, MedCompass, communication, and deadline requirements.
AHAC serves Alameda, Contra Costa, Fresno, Kern, Los Angeles, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Mateo, Santa Clara, and Sonoma counties.
